How to Create a New Consent Form Template and How to Sign It



To create a new consent form template, navigate to Patient → Configuration → Consent Form Template.


There are two types of consent form templates available in the system:

  1. Patient Consent Forms
  2. Patient Sale Order Consent Forms

  1. Patient Consent Forms

To create a patient consent form:

=> Click the Create button.

=> Enter the template name.

=> Check the Default option, if you want this consent form to be automatically added when creating a new patient.

=> Enter the body content of the consent form template.

You can include patient details in the consent form by using the available tags shown below. These tags will automatically populate the patient’s name and other relevant details when the form is printed.


Then, navigate to Patient → Configuration → Consent Form to create a new consent form.


Then, click the New button to create a new consent form. Next, select the desired consent form template using the Template button. All the details will be filled in automatically. Finally, click Save.


To create a sale order consent form, navigate to Patient → Configuration → Consent Form Template and create a new template specifically for sale orders.

There are two fields available for configuring a sale order consent form:


This field defines when the consent form will be applied to a sale order:

  1. Patient Frame – Applied when creating a glasses-type sale order with Frame Source = Patient.
  2. Proof of Delivery – This consent form is added to every sale order at the time of creation.

    Polycarbonate – Applied if any of the following conditions are met:

    • Patient age is less than 18.
    • The order has GLS Balance or Left GLS Balance checked.
    • No polycarbonate lenses are added to the sale order.
  3. All Glasses Orders – Added to all glasses-type orders.
  4. All Contact Lens Orders – Added to all contact lens orders.

B) Insurance Company

  • In this field, the user can select a specific insurance company.
  • If an insurance company is selected here, the order consent form will only apply to orders with that insurance company.
  • If the field is left blank, the consent form will follow the rules defined in the Order Consent Form field.


A) Inside the Patient Screen - The user can view all consent forms associated with that patient.


Inside Patient → Documents → Consent Forms, the user can view all consent forms associated with that patient.

If a consent form is linked to a sale order, the sale order number will be displayed on the right side.



B) Inside the Sale Order Screen - The user can view all consent forms related to that particular sale order.


To view consent forms for a sale order, go to the patient’s record, locate the desired sale order, and open it.



To sign a consent form from the backend:

  1. Click on the consent form record.
  2. Scroll down to the Signature section at the bottom.
  3. Click on the signature box.
  4. You can draw your signature directly in the box or upload a file (PDF, image, etc.) as your signature.


After completing the signature, the consent form status will be updated to Signed.
